The Florida Conrad 30 deadline gives physicians, employers and attorneys until 11:59 p.m. EDT Saturday, Oct. 31, to upload a complete waiver application to the state.
The Florida Department of Health Conrad 30 program page says packages are accepted only through the department’s FTP server. Applicants who do not already have access credentials are told to contact the Primary Care Office well before the deadline.
The program can sponsor no more than 30 physicians during the annual cycle. A complete package does not guarantee sponsorship, and this guide does not replace the controlling state rules or advice from an immigration attorney.
Florida Conrad 30 deadline and filing map
| Requirement | Official standard | Final review question |
|---|---|---|
| Deadline | 11:59 p.m. EDT Oct. 31 | Has the full package reached the FTP server? |
| Physician license | Clear, active Florida MD or DO license effective by Oct. 31 | Is the license unrestricted and active? |
| Work commitment | At least 40 hours a week of in-person direct patient care | Does the contract exclude administrative work and telemedicine from the 40 hours? |
| Employment term | At least three years | Are the start and end dates written into the contract? |
| Practice sites | Every location must be identified by physical address | Are all sites in Florida and supported by the required designation record? |
| Upload method | Department FTP server | Were credentials requested before the closing day? |
The official page says Florida uses the program to place physicians in federally designated health professional shortage areas. Applicants need a U.S. Department of State case number and must agree to live in Florida and treat patients only in Florida during the approved employment period.
Because the Florida Conrad 30 deadline applies to a complete package rather than a notice of intent, applicants should not treat a partially uploaded file as a placeholder.
Physicians eligible for the federal Health and Human Services exchange visitor program are not eligible for Florida’s Conrad 30 route. Resident, fellow, intern, house physician and similar registrations also do not satisfy the active Florida license requirement.
Employer and contract checks
Florida lists three employer routes. A sponsoring practice must qualify as a free clinic, be fully funded by state or local government without charging patients, or actively accept and bill Florida Medicaid. Every physician at a practice location must accept Medicaid; the Conrad applicant cannot be the only one.
The contract requirements are unusually specific. The state page requires:
| – signatures and dates from the physician and employer; |
|---|
| – at least 40 weekly hours of face-to-face patient care; |
| – a minimum three-year employment term; |
| – work to begin within 90 days after federal waiver approval; |
| – no noncompete provision; |
| – termination only for cause; and |
| – notice to the Primary Care Office before a planned termination. |
Missing language can require an addendum. A restriction that functions like a noncompete can also make the contract ineligible even when the document uses a different label.
For employers, the Florida Conrad 30 deadline is therefore also a contract-review deadline: unresolved site, schedule or termination language can affect whether the state accepts the package.
What the upload package needs
The department’s checklist includes the current sponsorship application, physician agreement, exclusivity attestation, one practice-location attestation for every site, the employment contract and addenda, shortage-area evidence, the physician’s curriculum vitae and federal immigration forms.
Specialists need the specialist addendum. A practice site outside a designated shortage area requires the Flex Addendum. The state warns that using an outdated form, leaving a field blank or submitting through a method other than the FTP server can make a package ineligible during the initial review.
During a final Florida Conrad 30 deadline review, the checklist should be matched to the current forms on the state page instead of a package saved from an earlier cycle.
Florida health professionals can follow other state program deadlines through The Daily Quill’s Health and Wellness desk. The site’s recent Florida professional training guide is a separate continuing-education opportunity and is not part of the Conrad 30 application.
The immediate task is mechanical: verify the active license, compare every contract clause with Rule 64W-1, confirm each practice-site record and upload before 11:59 p.m. EDT Oct. 31. The Florida Conrad 30 deadline page should be checked again before transmission, and applicants should retain proof that the complete package reached the state system.
